db: to the extent it makes sense to have a one byte size limit, make it 255 instead of 256 (Issue #96) Works around some index size issues on MySQL - http://mechanics.flite.com/blog/2014/07/29/using-innodb-large-prefix-to-avoid-error-1071/ .
